Class Audit & Blackboard Access Request Form

To request a CSUF Class Audit Form and access to the Blackboard site for a course you are auditing, please complete and submit the information below. You will receive by email, a pdf file of your completed Class Audit Form and your Blackboard user-id and password that allows you to access CSUF's electronic Blackboard. If you do not receive your requested information within 2 full working days following submission, please contact the OLLI office. If you have any questions concerning this form, please e-mail distance-ed@fullerton.edu.
